Vendor Risk Assessment Questionnaires: How to Design, Send and Review Them

A 300-question spreadsheet sent to every supplier produces fatigue, not assurance. Design questionnaires by tier, ask for evidence, and review answers consistently.

Key takeaways

  • Scope questionnaires by tier and service type.
  • Ask for evidence on the questions that matter.
  • Score consistently and record the reviewer’s reasoning.
  • Use clarification rounds instead of starting over.

Start from the risk, not the template

A cloud provider hosting customer data and a stationery supplier do not need the same questions. Tier vendors first, then select questionnaire modules: information security, cloud, privacy and data protection, business continuity, AI use, ESG and regulatory requirements.

Questionnaire modules by tier

TierTypical scopeEvidence expected
Tier 1 — criticalFull security, privacy, continuity, sub-contractors, exitCertifications, independent audit reports, policies, test results
Tier 2 — highSecurity and privacy core setCertifications or audit reports, key policies
Tier 3 — moderateShort security and privacy setSelf-attestation with selected evidence
Tier 4 — lowBasic information and contract termsContract and insurance

Writing good questions

  • Ask one thing per question.
  • Prefer closed answers (Yes / No / Partially / Not applicable) with a comment field.
  • Mark which questions require evidence.
  • Map each question to the control it tests — ISO 27001, your regulator or your own policy.
  • Avoid questions the vendor cannot answer without legal review.

Sending without e-mail chaos

Send questionnaires through a secure portal rather than as attachments. Vendors save progress, upload evidence against each question and submit once; answers then lock. Automatic reminders replace manual chasing, and every access is logged.

Reviewing answers consistently

Reviewers should accept, reject or request clarification per answer, with a note. AI can pre-screen: flagging contradictions (for example, “no third-party hosting” while the evidence names a cloud provider), missing evidence and expired certificates. Confirmed gaps become issues with severity and due date.

Scoring

Combine answer scores into a control-strength rating, then combine control strength with the inherent tier to obtain residual risk. Keep the method documented so auditors can reproduce it.

Evidence to request

TopicUseful evidence
Information securityISO 27001 certificate with scope, SOC 2 report, policies
Access controlMFA configuration evidence, access review records
Vulnerability managementRecent penetration test summary, patching metrics
Business continuityContinuity plan, latest test results
PrivacyData processing agreement, records of processing, sub-processor list
Incident responsePlan, notification timelines, recent exercise

Reducing vendor fatigue

Large vendors receive hundreds of questionnaires a year and answer the shortest ones first. Accept their standard reports where they cover your questions, ask only for what changed at reassessment, and send a pre-filled questionnaire based on last year’s answers. Shorter, sharper questionnaires come back faster and with better evidence.

Frequently asked questions

Should we use a standard questionnaire such as SIG or CAIQ?
Standard questionnaires help vendors reuse answers. Many organizations accept them for lower tiers and add regulator-specific modules for critical vendors.
How often should vendors re-answer?
Annually for critical and high tiers, every two years for moderate, and on major change or incident for all.
Can we import our existing Excel questionnaire?
Yes, modern TPRM platforms import questions from Excel and let you map them to controls.
