Vendor Risk Assessment Questionnaires: How to Design, Send and Review Them
A 300-question spreadsheet sent to every supplier produces fatigue, not assurance. Design questionnaires by tier, ask for evidence, and review answers consistently.
Spreadsheets and e-mailed questionnaires cannot keep up with hundreds of vendors. Here is what AI-driven TPRM software must deliver — and where human judgement must stay in charge.

Most third-party risk programs start with a spreadsheet register and questionnaires sent by e-mail. It works for twenty vendors. At two hundred, assessments stall in inboxes, evidence expires unnoticed, issues are tracked in a separate file and nobody can show a regulator the full history of a decision.
Regulators have raised the bar at the same time. Supervisors now expect a risk-based inventory of all third parties, proportionate due diligence, contract clauses, ongoing monitoring and tested exit plans — and evidence of each step.
| Activity | Manual approach | With AI |
|---|---|---|
| Intake and tiering | Analyst judgement, inconsistent | AI proposes Tier 1–4 from data access, criticality and materiality, with the drivers shown |
| Answer review | Read every answer line by line | AI flags weak, contradictory or unsupported answers |
| Evidence review | Open each PDF, check dates | AI reads certificates and reports, flags missing or expired evidence |
| Issue drafting | Copy findings into a tracker | Gaps become issues with severity, owner and due date |
| Reporting | Build slides each quarter | Analytics and AI-written board reports from live data |
AI is a reviewer, not a decision-maker. Good platforms keep tier overrides, risk acceptance and the final approve / reject decision with named people, require a second person for every decision, and log who did what. Regulators will ask for exactly that trail.
| Metric | Why it matters |
|---|---|
| Assessment cycle time | Shows whether due diligence is slowing the business |
| Share of vendors assessed | Coverage of the inventory |
| Overdue remediation actions | Discipline of follow-through |
| High and critical residual risk | Exposure outside appetite |
| Mean time to remediate | Speed of risk reduction |
If third-party risk is the immediate pain, start with a standalone TPRM product and make sure it integrates with your GRC risk register. If you also run compliance and asset processes in spreadsheets, a suite on one platform avoids building integrations between three separate tools later.
NAZZTEC AI TPRM automates this end to end — with AI assistance, segregation of duties, analytics and signed reports — and is available as SaaS, in your private cloud or on-premises. Our consultants design and run the process alongside it through our Governance, Risk & Compliance services.

A 300-question spreadsheet sent to every supplier produces fatigue, not assurance. Design questionnaires by tier, ask for evidence, and review answers consistently.

Tiering decides how much due diligence each vendor gets. Get it wrong and you either over-assess the stationery supplier or under-assess the cloud provider.

An asset register in Excel answers “what do we have?” badly and “who approved that change?” not at all. Here is what an ITAM platform must add.
